NYC Parks Lifeguard Course Requirements
After qualifying, in order to participate in the NYC Parks Lifeguard Course, we require you to provide us with each of the following items. All of these items will be collected from you on your first day of class. Without the Medical Note, you will not be allowed to participate.
Medical Note
You must provide an original medical note stating that you have been found to be in good physical condition and are able to participate in strenuous lifeguarding activities.
Your note will be rejected if:
- The person signing the note is not a Medical Doctor (M.D.)
- The note is not written in the English language.
- The doctor's stationery is not an original.
- The doctor’s name and office address are not on the letterhead.
- The doctor's registry or license number is not on the note.
- The examination took place more than thirty days before it was handed in to the Lifeguard School.
Passport Photos
You must provide two passport-sized photos taken within the last thirty days. You cannot wear glasses or hats in these photos.
Proof of Age and Citizenship
Examples of proof of age and citizenship include:
A government issued Birth Certificate, U.S. Passport, Alien Registration Card, Certificate of Naturalization, State-issued Driver Licenses, State issued Non-Driver ID's, or other documentation deemed to be acceptable to the Lifeguard School.
A Signature from a Parent or Guardian
If you are under 18 years old, please bring an application with your parent's or guardian's signature.
